Women produce testosterone mainly in the ovaries and adrenal glands, and some is also made when precursor hormones are converted in tissues such as skin, fat and muscle. Why Would a Woman Have High Testosterone?

How High Testosterone in Women Is Diagnosed
Diagnosis starts with the story. In clinic we ask when symptoms began, how quickly they changed, whether periods are regular, whether there is unwanted hair growth, acne, scalp hair thinning, weight change, fertility difficulty, pelvic pain, menopausal status and any medication or supplement exposure. A family history of PCOS, diabetes, early balding or adrenal conditions can add useful context.
Blood testing is usually central. Total testosterone is commonly measured first, but it should not be interpreted alone. SHBG helps estimate free androgen exposure, and some clinicians calculate a free androgen index. Depending on the situation, tests may include DHEA-S, androstenedione, 17-hydroxyprogesterone, LH, FSH, prolactin, thyroid function, HbA1c, lipids and sometimes cortisol-related tests.
Timing can improve reliability. If a woman has natural menstrual cycles, early follicular phase testing is often preferred for several reproductive hormones, usually around days 2 to 5. A woman who wants pregnancy soon will usually need a different plan from someone whose main concern is acne or facial hair and who wants reliable contraception. The right approach may involve GP care, dermatology, gynaecology, endocrinology, fertility services or a combination.
For PCOS-related androgen excess, lifestyle measures can be meaningful, especially where insulin resistance or weight gain is part of the pattern. Sustainable nutrition changes, regular resistance and aerobic exercise, improved sleep and reduced alcohol intake may improve metabolic markers and sometimes help menstrual regularity. This is not a blame-based message: PCOS is biological, and weight loss is neither easy nor the only valid treatment.
The combined oral contraceptive pill is often used when contraception is wanted and there are no contraindications. It can reduce ovarian androgen production, raise SHBG and improve acne or hirsutism over time. Benefits are not immediate; hair growth cycles mean hirsutism may take six months or longer to improve. Anti-androgen medicines may be considered when symptoms persist. Spironolactone is commonly used off-label for acne or hirsutism in some settings because it blocks androgen effects at the receptor and may reduce androgen production. It requires medical supervision, attention to potassium and kidney function in selected patients, and reliable contraception because of potential effects on a male fetus.[6]
Other options may include topical eflornithine for facial hair, dermatology treatments for acne, laser hair reduction or electrolysis for unwanted hair. These do not correct the hormone cause, but they can greatly improve day-to-day confidence while the medical plan takes effect. For scalp hair thinning, clinicians may assess ferritin, thyroid function, vitamin D, family history and consider treatments such as minoxidil where appropriate.
If fertility is the priority, treatment focuses on ovulation and metabolic health rather than suppressing androgens with contraceptive medication. Women with PCOS who are trying to conceive may be offered weight and lifestyle support if relevant, ovulation induction medicines through appropriate services, and assessment for other fertility factors.
